If you ask the blocking administrator to comment on this request, please place {{subst:Unblock on hold-notification | 1=Unblock-un}} on the administrator's talk page. Then replace this template with the following:

If you accept the unblock request, replace this template with the following:

If you decline the unblock request, replace this template with the following, replacing {{subst:Decline reason here}} with any specific rationale. If you do not edit the text after "decline=", a default reason why the request was declined will be inserted.

Usage
To use this template, place {{Unblock-un|user=new username|reason=your reason here ~~~~}} on your user talk page.
This template will place user talk pages in Category:Requests for username changes when blocked and Category:Requests for unblock.
