This template can be used, alongside {{user}} and related request information templates, to provide quick links for checking whether a user has ever been {{alert}}ed of the existence of discretionary sanctions for a topic.
For example, this template might be used at the beginning of an arbitration enforcement request:

The introductory text, Search DS alerts:, can be changed using |2=. To hide the text, declare it as empty: {{ds/log|Example|2=}} would produce,

The piped text in user talk history can be changed using |3=. The piped text in system log can be changed using |4=. |1= will default to Example (that is User:Example) if not declared.
